AV definitions updates stopped working after automatic update to ver 3.13.121240.574 :
“Update failed. AV engine is not initialized”
‘diagnostics’ found no problem.
reinstall didn’t help.

AV engine not initialized suggests that cmdagent.exe is not running and/or crashed.
Do you have any other security software installed that could conflict with CIS install?
No matter if Real-Time or OnDemand…
Can you check TaskManager to see if cmdagent.exe is running?
