This might have been proposed before, but how about incorporating bug reporting into the CIS software? Like this you could preformat all the fields you want the user to include, with an option to append a screenshot and/or file.
Maybe it would even be possible to add a forum login possibility within the software and let the form create new topics automatically (of course this would have to be spam proof and duplicate proof, maybe by generating an ID).
