When to use multiple domain names in an Exchange 2007 certificate?

Hi:
I have an Exchange 2007 server to whom I access from the Internet using a Public Domain Name and a Public IP with Outlook Web Access. Also this same server is accessed in the intermal network with a private IP and private local domain name with Outlook 2007. Do I need a certificate with these two domain names? If so, how do I request such certificate? ??? I have used IIS 6 to request a certificate but I only see a blank for only a FQDN.
